Howard tremendous insecurity (13+ years of therapy, little progress) has translated into surrounding himself with lesser creatures that are financially dependent on him, so that he has both control leverage and the upper-hand mentality. He also avoids hiring competent professionals who might challenge his self-centered ways and embarrass him. They must be inferior or it's a no-go. (Ben Stern surrogates like Pat and Mel are the only exceptions to this since Howard is still wanting for strong personal mentoring.)

Many successful but mentally-disturbed people create cults in their lives. I personally observed an example of one Oscar-winning actress and the cult of people she surrounded herself when I dated her niece. Bizarre birthday parties, meals etc.
